Starting November 1, 2005, alumni who want to apply for the Alumni Library Card or the Digital Library Service for Alumni no longer need to come in person to the Library or obtaining the application form via downloading or fax. The Alumni Data Form at http://www.alumni.cuhk.edu.hk/eng/contact/dataform.html is now available for alumni to apply for these services online anywhere and anytime. Alumni may log in the Alumni Data Form by inputting the name, graduation year, college, major, and the first 6 digits of the HKID Card Number; or by entering the username and password of the CU Alumni Free Email Forwarding. Once logging in, alumni may fill in the online form to apply for the above services. The payment for the service may be made in cheque and sent to the Circulation, University Library, The Chinese University of Hong Kong by postal mail.
