One set of the following documents should be submitted to the Student Services Centre (1/F, Benjamin Franklin Centre) or email to ritaleeom@eservices.cuhk.edu.hk by 5:30 pm on 12 September 2016 (Monday):

i.   A duly completed application form

ii.  A copy of CUHK Student ID Card

iii. A copy of HKID Card

iv. A copy of Report of Experiential Learning Activities (ELA Report) under the Student Development Portfolio (SDP)

Selection Procedures

i.  Applications will be reviewed by a selection panel on 23 September 2016 (Friday) (applicants should make themselves contactable and available for further arrangements)

ii. Successful applicants will be notified on 26 September 2016 (Monday)
