2016 Graduates' Application for "Report of Experiential Learning Activities"

Under the Student Development Portfolio (SDP), graduates* will be issued ONE Report of Experiential Learning Activities (ELA Report) free of charge upon their request.

Would all graduating students of 2016 who have ELA records in the SDP and who wish to receive the gratis ELA Report please send your request to the Student Activities Section by completing and submitting the application form at

https://cloud.itsc.cuhk.edu.hk/webform/view.php?id=1626277

by 5 August 2016 (Friday). You are required to provide all necessary information for verification and delivery arrangement. 

 

Successful applicants will receive an email confirmation in August 2016, followed by the delivery of the ELA Report by post.
