1) Criteria for recommendation: Students who have achieved a position within the top 10% of their peers at the same level.
2) Please upload a scanned copy of the completed form, attached letter and other supporting documents (if any)
on or before
• 27 March 2026 (for non-local students) or
• 24 April 2026 (for local students)

To applicant: Please submit an online application for CUHK SI to obtain the application no., then complete Part I
of the form. Next, pass it to your school principal to complete Part II and submit it for you.
