如有興趣享用生日優惠，請於您的生日月份或前一個月內填妥此表格。校友事務處會在核對資料後，以電郵方式發送參考編號給您。 收到生日電郵後，請致電+852 3723 1234，並於預訂時引用生日電郵內所提供的參考編號。 Please complete this form during your birthday month or the month prior if you are interested in the birthday offer. After verifying your eligibility, the Alumni Affairs Office will send the reference number to you via email. Upon receiving the reference number, advance booking is recommended.
