The personal data provided to Wellness and Counselling Centre (WACC) will be used by WACC for the handling of your enrolment in our programme(s)/workshop(s) and providing services. The provision of personal data is voluntary. However, if you do not provide sufficient information, there may be case that WACC cannot provide the needed service(s) to you. You have the right to request access to and correction of your personal data held by WACC. Photo Taking and Video Shooting
Photos will be taken during the workshop. These images may be used for programme promotion on our instagram accounts and CUHK webpages. Promotional materials containing the photos may also be used for campus-wide promotion. If you would prefer not to be photographed, please send an email to wacc@cuhk.edu.hk by two working days before event date and remind us again during the photographing, thank you.

Statement of WACC Certified QPR Gatekeeper Training for Suicide Prevention: Registration and Attendance Rules
Upon completion, you will become a certified QPR Gatekeeper and receive a certificate issued by the QPR Institute.
Participation
- 100% attendance is required for certification. If you cannot participate the 2.5 hours course, we suggest you to consider joining the next round of Certified QPR Gatekeeper Training at your convenience.
Tardiness and Withdrawal
- Lateness, early leave or leave taken at the participant’s own discretion will be counted as absence which will result in forfeit of your deposit and incompletion of the course. For withdrawal of the workshop within three working days before the workshop commencement, deposit will be forfeited. For any reason participants who are withdrawing from the workshop, deposit reimbursement can only be considered based on proof of medical certificates.
- Latecomers exceeding 15 minutes will not be allowed to enter, and deposit will be forfeited.
- Participants are not allowed to take photos or record videos during the event.
- Participants are not allowed to copy or share any program materials, and the instructors reserve the right to decide to share in either paper or digital format.
In the case of any disputes, the centre reserves the right of final decision.
