註：我們已優先為已報名同學配對，是次延長報名未必能完全滿足於原定截止報名日期前報名的同學。
Note: Priority matching has already been arranged for students who registered earlier. This extended registration period may not fully accommodate all new applicants who did not applied before the original deadline.

重要資訊：

1. 書院將於以電郵形式通知報名結果，並須於收到電郵三天內回覆確認，否則視放棄論。請時常查閱電郵，亦建議檢查「垃圾郵件匣」。成功申請之學員必須參與9月19日(以廣東話進行) 或 9月22日(以英語進行) 其中一場學員簡報會，否則同樣視作放棄論。於9月22日之後報名的同學，將會於10月10日前收到簡介文件，同學必須回覆電郵表示「已閱」。

2. 學員將會於啟動典禮前獲知初步配對結果(學長之背景，但不包括姓名和畢業年份)，學員可於電郵發出兩天內申請更改，書院會盡力處理，唯書院擁有最終決定權。

下列表格分四部份。甲部將會轉交獲安排往相同大組之學長及學員、以及「新亞書院學長計劃統籌委員會」於本屆計劃所有相關活動中使用於本屆計劃所有相關活動中使用；乙、丙及丁部只會用作書院於本計劃之內部分析用，不會向第三者提供。填寫本表格時，請於適用方格填上剔號。

Please read the "Introduction to NAC Mentorship Programme" and "Guide to NAC Mentorship Programme" before filling in the form.

1. The application results will be sent to you by email. Please check you inbox regularly and also look for the email in your email "junk box". You MUST reply to the email to confirm your participation to the programme in 3 days upon the receipt of the confirmation email. Successful applicants MUST attend one of the briefing sessions which would be held on 19 Sep 2025 (in Cantonese) or 22 Sep 2025 (in English).
Students who apply after 22 September will receive the briefing documents by 10 October. They are required to reply to the email with a confirmation stating ‘Read’.

2. The preliminary matching results will be announced to the mentees before the Kick-off Ceremony (A brief background of your mentor will be sent to individual mentees). You may make appeal to the matching result. The College will do our best to help you. But the College reserves the rights of final decision.

This application form consists of 4 parts. The first part would be sent to your groupmates (mentors and mentees of your "big group"); and the Executive Committee of New Asia College Mentorship Programme. The later 3 parts would only for College internal use.

    If you cannot commit to fulfill these requirements, you will not be able to complete this Programme.

    填妥本表格後，書院會盡力為你找適合學長，但不保證能提供完美配對，更有可能未能為你配對。我們採取報名先到先得為原則。
    The College will try to give you a best match after you have completed the application. But we do not guarantee to provide the best match, there may be cases that we cannot provide a matched result. The matching is processed on first come first served (by application time) basis.

  • 本計劃要求學員與學長至少會面兩次 (一學期一次)。如同學因交流計劃而離港，請學長安排在同一學期內見面，並準備以網上會議形式保持聯絡。同時我們建議雙方以任何形式(包括電子方式)接觸每月一小時或全年總共八小時。請確認您可以此為目標：
    You must meet your mentor at least TWICE (once a semester). If you are participating in an exchange programme, please meet your mentor in one semester. And please continue the communication electronically. We recommended setting aside about 1 hour each month or around 8 hours over the year for interactions specific to each mentoring relationship. Please confirm that it is possible for you to make this time commitment to your mentee and the programme.
    本計劃要求學員與學長至少會面兩次 (一學期一次)。如同學因交流計劃而離港，請學長安排在同一學期內見面，並準備以網上會議形式保持聯絡。同時我們建議雙方以任何形式(包括電子方式)接觸每月一小時或全年總共八小時。請確認您可以此為目標： You must meet your mentor at least TWICE (once a semester).     Groups that being able to meet in the Kick-off Ceremony usually have fruitful results at the end of the programme. You are required to attend the Kick-off Ceremony on 1st November (Sat). If you really cannot make it, please submit the absent request form to the college and also notify your mentor. Please also arrange your first online meeting with ALL groupmates within one week after the ceremony.
    一般而言，能出席啟動典禮的組別，最終都有良好成果，請你務必出席11月1日(六)舉行之啟動典禮。如因事未能出席，請提交須出席活動缺席表格通知書院及學長，並在一星期內與全組人網上會面。 Groups that being able to meet in the Kick-off Ceremony usually have fruitful results at the end of the programme.
