1. 填表前請先瀏覽書院網頁 (https://www.uc.cuhk.edu.hk/student-hostels/hostel-news/) 有關校友租用聯合書院住宿申請須知。
Please read the notice to applicants for United College Hostel Rental for Alumni on the College website (https://www.uc.cuhk.edu.hk/student-hostels/hostel-news/) before filling in the form.

2. 宿費以入營人數為單位計算，租住雙人房/三人房每人每晚為港幣268元。聯合校友可獲七折優惠﹝每人每晚為港幣188元﹞，
The accommodation fee is calculated based on the number of campers. The rate is HK$268 per person per night for a double room or triple room. 30% discount will be offered to UC Alumni (HK$188 per person per night).

3. 租用校友團體入住人數最少為10人，恕不招待十二歲以下營友。
Each alumni group should comprise at least 10 participants. Guests under 12 years old will not be accepted.

4. 完成網上申請後，申請人將透過電郵收到一份租用聯合書院住宿申請確認書。
Applicants will receive a confirmation letter for UC hostel rental application via personal email after completing the on-line application.

5. 書院將不接納上述營會負責人以外之人士查詢或處理租用團體營會事宜。
The College will not accept another person other than the above person-in-charge to make enquiry and process the application.

6. 此表格所收集的資料將用以處理有關的申請，所提供的資料於無需保留時將全部銷毀。
The personal data provided on this form will be used by United College for the sole purpose of processing this application. All information provided will be destroyed when it is no longer required.

7. 如在遞交此表格後需要更改資料，請電郵至聯合書院學生輔導處(電郵：carmankoo@cuhk.edu.hk)
For amendment of data after submission of this form, please email to the UC Dean of Students' Office (email: carmankoo@cuhk.edu.hk)

8. 如申請獲批准，請於指定日期前繳交宿費(沙田香港中文大學聯合書院學生輔導處古女士收)
請將款項直接存入香港中文大學恆生銀行賬戶：293-005-005-018，並將存款記錄寄到香港新界沙田香港中文大學聯合書院。
If the application has been accepted.
Please make direct deposit to CUHK Hang Sang Bank account: 293-005-005-018 and send the deposit slip to United College, The Chinese University of Hong Kong, Shatin, New Territories, Hong Kong
