Personal Information Collection Statement
The personal data provided to Wellness and Counselling Centre (WACC) will be used by WACC for the handling of your enrolment in our programme(s)/workshop(s) and providing servies. The provision of personal data is voluntary. However, if you do not provide sufficient information, there may be case that WACC cannot provide the needed service(s) to you. You have the right to request access to and correction of your personal data held by WACC. Enquiries concerning the personal data provided, including data access and data correction requests, should be addressed to Manager, Wellness and Counselling Centre, Office of Student Affairs, 2/F., Pommerenke Student Centre.

Statement of WACC MHFA Course Registration and Attendance Rules
The MHFA is a standardized training course which requires participants to complete 12 hours of training and pass the written test in order to receive the certificates. Through the act of joining this course, you agree to comply with the rules, regulation and final decisions of your instructors. Please acknowledge that we hold participants to the standard as required by the Mental Health Association of Hong Kong.
Participation
100% attendance is required for certification. If you cannot participate all 4 sessions, we suggest you to consider joining the next round of MHFA courses at your convenience.
Tardiness
Lateness, early leave or leave taken at the participant’s own discretion will be counted as absence which will result in forfeit of your course fee and incompletion of the course. For medical reasons with medical proof submitted within 3 days, 1 chance of make-up class can be granted. It should be completed within 6 months, otherwise previous attendance will be nullified beyond this point.
Withdrawal
For withdrawal of the course within three working days before the course commencement, course fee will be forfeited.
