Application for Suspension of studies (23-24) -Exchange

1. This form is specifically for QFIN/QFRM students who wish to suspend their studies for the purpose of participating in an exchange programme. If you want to suspend your studies to take up an internship, please use the following link https://cloud.itsc.cuhk.edu.hk/webform/view.php?id=13679470.
2. Once you have received a letter of offer from OAFA, please prepare the following documents:
a. Suspension of Study Application form for exchange program
b. Letter of offer from the host university (This is a mandatory requirement. Please make the application until you have received the letter of offer. We cannot proceed with your case without this document.)
c. Study plan in Excel format
d. Your most updated transcript
e. (For QFRM students only) Study plan in Word format, ensuring that the information is consistent with the Excel file.
f. (For QFRM students only) Declaration of Programme Completion
3. The Suspension of Study Application form, Study plan in Excel format, Study plan in Word format, and the Declaration of Programme Completion can be downloaded using the following link https://drive.google.com/drive/folders/16rYGXN4a1sGyI9UsEJ54G9NHNGGArN_n?usp=sharing. Please refer to the sample files for guidance on how to fill in these forms.
4. Please save documents “a-c” as a single PDF file and submit this document along with documents “d-f” when completing this form. Additionally, please send all documents and your request to Rika at qfin@cuhk.edu.hk.
5. The processing time for this application is typically 7-10 working days. However, failure to submit a complete application or incorrect information will delay your application.
6. Please be advised that this application is solely for requesting a suspension of study. Any other matters, such as credit transfer, deferment of the study period, or exceeding course limits, will not be taken into consideration or approved through this application, even if you mention them in your study plan.
7. The study plan is intended solely for planning purposes. Approval of your application does not imply that no changes are needed in the study plan to ensure timely graduation. Students are strongly encouraged to review CUSIS requirements and course availability to facilitate a seamless graduation process.
