Please read the notice to applicants for United College Hostel Rental for Outside Groups on the College website (https://www.uc.cuhk.edu.hk/student-hostels/hostel-news/) before filling in the form.

2. 宿費以入營人數為單位計算，租住雙人房或三人房每人每日港幣250元
曾於2017年,2018年,2019年租用本院學生宿舍的團體可獲九五折優惠
校內部門可獲八折優惠
書院屬下團體及院系會預營可獲七折優惠

The rate is HK$250 per person per night for double or triple rooms
5% discount will be offered to groups who have used UC hostel facilities in 2017,2018,2019
20% discount will be offered to CUHK units
30% discount will be offered to group affiliated with College / departmental o'camp pre camp

3. 資料不足或逾期遞交之申請概不受理。
Incomplete forms or late applications will not be accepted.

4. 完成網上申請後，申請人將透過中大學生電郵/電郵收到一份租用聯合書院住宿申請確認書。
Applicants will receive a confirmation letter for UC hostel rental application via personal CUHK email after completing the on-line application.

5. 書院將不接納上述營會負責人以外之人士查詢或處理租用團體營會事宜。
The College will not accept another person other than the above person-in-charge to make enquiry and process the application.

6. 此表格所收集的資料將用以處理有關的申請，所提供的資料於無需保留時將全部銷毀。
The personal data provided on this form will be used by United College for the sole purpose of processing this application. All information provided will be destroyed when it is no longer required.

7. 如在遞交此表格後需要更改資料，請電郵至聯合書院學生輔導處(電郵：carmankoo@cuhk.edu.hk)
For amendment of data after submission of this form, please email to the UC Dean of Students' Office (email: carmankoo@cuhk.edu.hk)

8. 如申請獲批准，請於指定日期前繳交宿費(沙田香港中文大學聯合書院學生輔導處古女士收)
1) 外界團體及學生組織：劃線支票(抬頭請付「香港中文大學」，支票背面請列明申請編號及營會名稱)；
2) 大學校內教學/行政單位：內部轉數紙(請列明申請編號)
If the application has been accepted.
1) External organizations / Student Organizations: Please send a crossed cheque (payable to “The Chinese University of Hong Kong” and quote our reference number at the back of the cheque) to Ms Carman KOO of the Dean of Students’ Office, United College, The Chinese University of Hong Kong, Shatin, N.T.
2) CUHK teaching/administrative units: Please send the Interdepartmental Transfer Form with the booking reference number, to Ms Carman KOO.
