We cordially invite you to participate in an online survey entitled ‘Needs Assessment and Expectations of the Cochrane China Network: A Multicentre Study’ initiated by Cochrane Hong Kong, The Nethersole School of Nursing, Faculty of Medicine, The Chinese University of Hong Kong. In the following paragraphs, the key information of this research is summarised for you to understand why this survey is being done and what it would involve for you. Please take the time to read this information carefully and feel free to contact the research team if you have any questions.

Why this survey is being done?
為什麼要進行這項調查？
The overall aim of this study is to understand the needs and expectations of frontline healthcare professionals, managers, health policy makers, researchers, and academics in mainland China and Hong Kong on the Cochrane China Network. Cochrane is an international, independent, not-for-profit organisation dedicated to synthesising up-to-date and high-quality health-related research evidence and making it readily available worldwide. A Cochrane China Network consisting of the Cochrane China and eight affiliates from eight cities/regions in China, including Chengdu, Beijing, Chongqing, Lanzhou, Ningbo, Shanghai, Wuhan, and Hong Kong (https://china.cochrane.org/network-china) has recently been established to better support evidence-based health care practice and policy decision-making in the region. To make the best use of resources and to devote our efforts to the most needed areas, we need to understand the experiences of stakeholders, including healthcare professionals, managers, health policy makers, researchers, and academics, in using research evidence so as to identify their needs and expectations in implementing evidence-based practice and making policy decisions. 這項調查的總體目標是瞭解中國內地和香港的臨床醫護專業人員、管理人員、醫療政策制定者、研究人員和學者對Cochrane中國協作網的需求和期望。 Cochrane是一個國際的、獨立的、非盈利的組織，致力於綜合最新的、高品質的與健康相關的研究證據，並使其在世界範圍內被應用。由來自成都、北京、重慶、蘭州、寧波、上海、武漢、香港的Cochrane中國中心和8個附屬機構組成的Cochrane中國協作網（https://china.cochrane.org/network-china）最近已經成立，以更好地支持中國循證醫療實踐和政策制定。為了充分的利用資源，並將我們力量投入到需求最迫切的領域，我們需要瞭解持份者，包括醫護專業人員、管理者、醫療政策制定者、研究人員和學者在使用實證方面的經驗，以確定他們在循證實踐和政策制定方面的需求和期待。

The views and opinions you provide through the online survey will be used to inform what and how we should support our stakeholders on evidence-based healthcare practice and policy decision-making in mainland China and Hong Kong. How do I know if I am eligible?

You are eligible to take part if you are working in mainland China or Hong Kong, and belong to one of the following stakeholder groups: healthcare professionals, managers, health policy makers, researchers, and academics who need or use research evidence in practice or making decisions. 如果您在中國內地或香港工作，且屬於在實踐或決策中需要或使用研究實證的醫護專業人員、管理人員、衛生政策制定者、研究者和學者，則您符合本研究的參與資格。
What does taking part involve?
如何參與這項調查？
Participation will involve taking part in an online survey. Questions will inquire about your experience in finding and using research evidence and your needs in training, research areas, knowledge translation, and expectations of the Cochrane China Network. It will take around 10-15 minutes to complete the survey. You do not need to give your name or any identifiable information, and you may leave any questions not applicable to you blank. By taking part in this survey, you are agreeing to allow us to publish the research findings with de-identified data, where it will not be possible to link you as an individual to any of the findings. 您將透過完成線上調查的方式參與本調查。您需回答的問題將包括您查找和使用研究實證的經驗，您對循證培訓、循證研究領域、知識轉化等方面的需求，以及對Cochrane中國協作網的期望。完成調查大約需要10-15分鐘。您不需要提供您的姓名或任何可識別身份的資訊，對任何不適合您的問題，您可以不予作答。您參加本次調查，即表示您同意我們採用調查資料發表相關研究結果，研究結果將不包含任何可識別您身份的資訊。

Your identity will be kept confidential. What will happen to the findings of this study?
