Notes for applicants 1) Schedule and mode Schedule: 28 Sep 2024 to 25 Oct 2025 Mode: face to face mode supplemented by online workshops between courses. 2) Entry Requirements Applicant should - hold a Bachelor degree from a recognized university or equivalent professional qualification; - have completed an 8-week Mindfulness-based Cognitive Therapy for Life course as a participant, led by a teacher in-person or live online, within the last 18 months. If one session was missed, the applicant is still eligible for Teacher Training (TT). However, if more than one session was missed or it has been longer than 18 months since completing the course, it is recommended to retake the 8-week course before pursuing TT - have regular personal mindfulness practice, ideally for at least a year - have knowledge and experience of the population and context where you would like to teach, for example, workplace, education, criminal justice, physical or mental healthcare, or general population - have some knowledge and experience of working with individuals and groups access - suit technology, internet capacity, space, privacy and the technical ability to join an online training programme via Zoom with video for online workshops - sign the OMF teacher training agreement and agree to abide by OMF guiding ethical principles 3) Course Terms and Conditions: (1) 100% Attendance Requirement Students are required to attend all classes without exception. Absences may result in the student's inability to complete the course successfully. (2) Course Completion and Makeup In the event that a student is unable to complete any of the three courses offered in the current academic year due to unforeseen circumstances, they will be required to make up the missed course(s) in the following academic year. The student will be responsible for paying the tuition fees associated with the makeup course(s). The makeup course(s) will be charged on a per-course basis. (3) Final Decision Authority The course admin team reserves the right to make final decisions regarding any matters related to the courses, including but not limited to scheduling, content, make up class arrangement and assessment. Please review and acknowledge your understanding and agreement to these terms and conditions before enrolling in the course. 4) Submission of Application Form After submitting the online application, supporting documents should be sent to sphpc.mbct@cuhk.edu.hk via email and to the following address by post with an application fee of HK$300 (A crossed personal cheque made payable to: "The Chinese University of Hong Kong"). Course Administrator of "Professional Certificate Programme in MBCT Teacher Training" Room 426, 4/F, School of Public Health Building, Prince of Wales Hospital Shatin, N.T., Hong Kong 5) Documents to be submitted (1) Photocopy of identity card (via email + by post) (2) Copies of qualification certificates (including Professional Qualifications, Highest Academic Qualifications, 8-week MBCT-L programme attendance certificate and/or other mindfulness programmes attendance certificate, for example, 8-week MBSR/MBCT etc.(via email + by post) (3) an application fee of HK$300 (A crossed personal cheque made payable to: "The Chinese University of Hong Kong"). (by post) Original documents must NOT be sent. 6) Application Deadline The application form and all the supporting documents should be received by the Course Administrator on or before 2 July 2024. 7) Selection Process Both CUHK and OMF will review the submitted documents. Shortlisted candidates might be invited for an interview. In the selection process, we will consider both the suitability of the applicants and the date of submission. When applicants have similar levels of suitability for the position, priority may be given to those who submitted their applications earlier. This includes arranging interviews based on the order of application dates as a reference. Applicants will be notified of the application result when the Programme concerned has made its decision. 8) Tuition Fee Payment Tuition fee should be paid by cheque within 2 week after confirmation. The tuition fee, once paid, is non-refundable or non-transferable. Payment(s) should be made by a crossed cheque payable to "The Chinese University of Hong Kong".
