Please declare your health information to facilitate our event arrangement under the current social distancing measures. Priorities will be given to registrants who received at least one dose of COVID-19 vaccine in order to fulfill the Type ‘D’ mode of operation on catering businesses by the Government.
I have received/will receive the first dose of the new crown vaccine before the event and agree to present the vaccination record for verification.
I agree to use the "Leave Home Safe" mobile application to record my entry and exit to the venue for this event.* (In order to comply with government requirements, all participants must abide by this rule before eating or drinking in the venue.)

IMPORTANT NOTICE:

Deadline of registration: 13 September 2021 (Monday), 17:00. In accordance with the Type ‘D’ mode of operation regulation implemented on catering businesses by the government, special registration arrangement will be made as below:

For registrants who received at least one dose of COVID-19 vaccine before the event date, registration status will be notified by email within 3 working days on a first come, first served basis.

For registrants who are yet to receive COVID-19 vaccine before the event date, registration status will be notified by email no later than 14 September 2021 subject to availability under Type “D” mode of operation.

No cancellations or refunds will be made except for registrants whose registration were declined due to the limited capacity for participants without vaccination under Type “D” mode of operation. Refund will be processed within 2 months after the event date. No walk-ins will be accommodated.
