The CUHK Jockey Club Community Primary Care Programme: The Start of Integrated Multidisciplinary Primary Care

The programme which aims to integrate medical and social care needs in providing multidisciplinary healthcare and bridging local health initiatives to those most vulnerable has formally started in September 2012.

The fourth floor of the Lek Yuen Health Centre has completed the facelift in June 2012 to provide a multidisciplinary hub for the provision of integrated bio-psycho-social care to socially disadvantaged patients. A core team of clinicians, nurses, social worker, physiotherapist and dietician are ready to provide services and activities to impact individuals and their families.

 

Patient recruitment has commenced in mid-September 2012. These patients will receive longer clinical consultations, participate in health related activities such as health talks and physiotherapy workshops, and obtain advice from social worker when necessary. Furthermore, patients will be encouraged to participate in social activities delivered by the core team such as watching movies with other participants to extend their social network. Health assessment on patients will be done to test the feasibility and acceptability of this new model of primary health care delivery to the public and health professionals.

 

In addition to addressing the needs of the socially disadvantaged, the programme also provides health education for the general public in the community. General health assessment was made available to the public in a health exhibition held in November 2012. Immediate health advice was given to almost a thousand participants by clinicians and registered nurses during the activity. More exhibitions and health seminars will be held in future to educate the general public on the concept of family doctor and knowledge on health promotion and preventive measures to enhance optimal self-care.

 

By providing multidisciplinary healthcare to those most vulnerable and educating the general public on preventive measures to enhance optimal self-care, it is envisioned that the delivery of primary care for the community in Hong Kong will improve in the near future.
